I'm in need of a sample auditors evaluation. This evaluation serves as a feedback wherein the auditee can evaluate the performance of the auditor.

The attached form is confusing.

Does the auditor prepare a checklist? Does the auditor submits report within 24 hours?

How do you expect to answer these questions with "Poor, Fair, Good or Excellent"?

Better to have a Yes or No column rather than present subjective options that varies per auditees perception.
