I don't know of a book, but if you do a search here in the Cove on Corrective and Prevention Action you'll probably get some good links to some great information. Real world information...and that's better than a book! :)

Try doing a search in the Cove for "root cause analysis" and "corrective action"...I'm certain loads of links will pop for you to peruse and full of valuable information.

It is entitled,"How to Design a CAPA system for FDA regulated industries"
It a little handbook with very good information it.It helped me a lot.
