Does anyone have a completed identification tool from the AIAG that they would share? I have looked in the file section, but with no luck.

I don't remember seeing a completed one here, and I've never used the tool. Maybe by my bumping this up someone here will see this and help out.

I don't have a completed one, but the form, available for free download from AIAG (see link below), includes detailed instructions and examples, and seems to be pretty self-explanatory.
