We already have several documents available here in the Cove on your topic. You can try using the "search" button in the navigation bar near the top of the page. You may find something which will serve your purpose or can be adapted to serve your purpose. We are entering the weekend, typically slow here in the Cove. If you haven't found what you are looking for by Monday, come back and add a new comment to your thread to bring it near the top of the "last 24 hours" list which most regular Covers seem to use for their first look at new posts. Making your own form is usually a matter of understanding what is realistic in the organization, rather than trying to mold all your people to a generic form.
