I want to have a 2-hr exam after a in-company PPAP training.
Can anyone give me a template of a PPAP exam paper?

2 hour test time would indicate very in-depth testing -- most companies wouldn't require that kind of testing on such a narrow topic.

If you proceed with such a test, you will likely have to devise your own questions. The PPAP manual, customer-specific requriements, and your own procedures will provide information for many possible questions. Those questions can be formulated in the framework suggested by the answers to the questions below:

Who is being tested? Persons with PPAP responsibilities or internal auditors?
Will customer-specific PPAP requirements be included? If so, which customers?
Open book or closed book?
Format of questions: multiple choice, true/false, essay, mixed?

I agree; two hours seems a bit severe, and if you've devised specific PPAP training, you should also develop a test method based on it, and not a generic template. Each company has its own ways of meeting PPAP requirements.

Is this a test for new employees or a new PPAP program you're trying to implement?

If your internal auditor and PPAP responsible have already been preforming their PPAP job functions, perhaps continuous training is in order.:read:

Most of them are people who have PPAP experience.

To be honest, I don't have very much knowledge on PPAP. I am assigned to prepare a exam paper for the coming PPAP training. So, I want to know what kind of question should be asked? Are there any typical questions? A set of mc, short questions and long questions for my reference would be most welcomed.

I am sorry I do not understand what you want to do.
An exam in PPAP closed book to me is meaningless. There is no reason why anyone should learn by rote the levels of submissions etc.
The handbook is, or should be available for all who need it. When you say PPAP do you really mean an understanding of MSA and CPk and other tools not just on how to collect the various pieces of paper.
Please clarify why you need this exam and also why you as

I work as a supporting staff to my team. My teammate asked me to prepare a set of questions for the coming PPAP training course. We will have a exam after the training, that's why I need to prepare a exam paper. The exam should be open book, I made a mistake.
