You don't tell us what the "self appraisal report" is expected to include, nor whether the "self" is you personally or the system in general. If it's the latter, there must be some criteria the system is supposed to be evaluated against, no? List the criteria, and tell how your system satisfies them.
gpatel 26th June 2006, 11:22 AM I am attaching a file which has all the points and in details what is required.

You didn't attach the file, but if you already know what's required, what is your question?
Added in edit: The OP informed me via PM that he went back and attached the document he had referred to.

Is this a Supplier Self Audit (or equivalent) given to you by a prospective customer?
gpatel 26th June 2006, 11:41 AM It is a self audit and reporting for proving the quality management on paper reports by external body which I am unable to explain or I can say I can not prove it on papers.

We know nothing about your business or how it's conducted, or what role you play in it. The questions on the document you attached are straightforward, it seems. If you're not qualified to answer them, why not get some help from a coworker?
gpatel 26th June 2006, 11:49 AM Thank you for the reply. But the problem is I am the only handler or (I'm the MR, Prod. Exe, Sales & support Exe, Marketing exe. etc .etc.) of my quality management system, and no other official staff who can help.
If you can in brief let me know that in general what is to be written for the points and then i can write about the same with the supporting documents and records which i have for my management and the system.

Meh:truce: . I don't want to be rude, but let me put it this way. You are there, working in the company in question. I am here, 10,000 miles away, and I have no idea what you're doing. If you can't answer the questions, how is anybody else supposed to be able to?
Perhaps someone here who's completed such report will weigh in and offer some guidance, so stay tuned.

Wow - you are 25 and are the executive of the QMS, Production, Sales and Marketing?

Is your company ISO or TS registered?

I have to agree with Jim on this. If you ca't respond to the questions and you work there, then how can any of us help you?
gpatel 26th June 2006, 01:23 PM Dear Mr. Jim & Mr. Cari,
I am sorry i forgot to give my web address. Where anyone can know about us. it is h**p://www.metro-rajkot(dot)com. We are induction furnace manufacturer with total workforce of 10 in workshop and 2 people in management & development of products and 1 person in administration.
We are ISO 9001:2000 certified and have a well established ISO system running very smoothly and have designed the best of inductions with almost zero faults atleast untill 4-5 years of operation on site.
The main problem being we are not able to represent ourselves on paper.

I don't presume to speak for Mr. Cari, but let me ask you a question: Do the people working at your company have the proper training, experience and skills to be able to do their jobs competently? If the answer is "yes," how do you know it? What evidence is there? If the answer is "no," or you have no evidence, how did your company get ISO registration?
Al Rosen 26th June 2006, 02:24 PM Maybe it is simpler than you perceive it to be. Which of these questions do you find most difficult and why?

Maybe it is simpler than you perceive it to be. Which of these questions do you find most difficult and why?Yeah, let's try that. I can tell you how I'd answer the question for my company - and even though your company might not do the same thing, it might be helpful to see how I would answer.
A lot of the questions are the same questions that your registrar should be asking you during audits. How would you answer them?

Jim Wynne 26th June 2006, 02:58 PM Having fun?
Barrels :lmao:
ralphsulser 26th June 2006, 03:39 PM Are you reading my mind again?
After reviewing the questions, I thought the same thing. Sounds a lot like audit questions which should have already been answered somewhere during the ISO9001 certification process. Except for the financial questions. I don't think it is anyones business about your profitability, etc. Unless you are a public traded company.
gpatel 2nd July 2006, 05:22 AM Thank you all for your suggestions. I have finally completed the report from all the suggestions and few resources on the cove.
Once again Thank you all.

Welcome,
Like the previous user, you are not helping yourself or us by not giving enough details for anyone to offer suggestions. What is the purpose of the appraisal? Against what criteria? - Some standards such as ISO 9001?
In view of the urgency, I will direct you to our 'post attachments li (http://elsmar.com/Forums/fileslist.php)st' where you may be able to find some reference. This is what you get by using 'self' as a keyword (http://elsmar.com/Forums/fileslist.php?mode=allfiles&sortby=filename&pageamt=2&criteria=self)for searching.
ngohrvinet 25th June 2009, 04:53 AM Well, This file is very useful
Apart from that, we can visit self appraisal sample (http://www.humanresources.hrvinet.com/sample-employee-self-appraisal-form/) for reference.
