Currently I am working in purchasing field, my boss instruct me to make Monthly Supplier Perfomance. I had tried to search in this forum and found some samples of forms. We need form samples in excel file which contain delivery rating, quality rating, price rating include index ussualy used to.
If anybody has, please kindly attach your file here.

Have you looked through this?

Look through those then develope your own in excel to satify your requirements.
