I am new to this forum.
We are a small family business manufacturing medical devices for a big company. We do the machining on CNC Mills and Lathes and we also have some outside processes (i.e. anodizing, passivating, etc.) They require a PFMEA from us.
Would anyone have a example to get us started.

Let's start from the beginning:
Have you tried searching for your topic both here in the Cove and on Google?
Do you have an idea of what a FMEA (Failure Mode & Effects Analysis) really is? If not, search that, too.
There will be a list at the bottom of this thread of threads on the same or similar topics - read through them.

We'll be interested in hearing how it all works out for you when you learn how to do this yourself instead of blindly copying someone else's template. Knowledge is always an element of power!

If your not familiar with PFMEA's I would suggest taking a class on it. I took a one day course a few years back and I'm not sure if I could have picked up everything just from reading "how to" materials.

Thank you for the information.
I have done some DFMEA in the past.
I now have the necessary information to do the PFMEA and also I just obtained the DFMEA from the customer for that product.
