Hi Carol,
It is not the same.
Right now we want to implement a "temporary process change" procedure. Through this form we want to be able to approve changes in a same day fashion.
I'm just looking for a template.

Do you have a "Permanent" process change form? "KISS IT" - add a check box to designate a "temporary" or "permanent" change & use only 1 form.

One way to think of this is in the light of an ECN (Engineering Change Notice) or a Request for an ECR (Engineering Change Request).
In short, who (which department, if you will) is ultimately responsible for approving process changes (temporary or permanent)?

Mumus, I would question why this form is needed. Many times forms like these allow people to "do what they want". It may not be but i would certainly question the person wanting it and set clear guidlines and approval process for it's use.

If you feel that you must have a form to document a temporary change, then simply create a form that gives a date range for the process change, along with a discription of how to temporarily perform the process, a name of someone who is responsible - the "go to" person, and maybe a description of why you are changing the process.
We do this kind of thing through "trials". the trials are documented within the minutes of our core quality group. It all depends on what you have said you are going to do. It doesn't have to be complex, it just needs to work for you.
