Is there any specific rating table for services? I mean, I know rating tables for process, design, system and machine FMEA, but I´ve never heard about a special table for services like software implementation or sampling requests....

I have not seen a rating table nor have I heard of one. You could probably make your own that makes sense to you. Perhaps by "bumping" this someone will chime in with some suggestions.
