Each Product/Process Charateristic should be a separate line item. (In other words, there should not be an entry on the same line combining the two) Each Product/Process Characteristic needs to have a Control Method filled in (two are missing). The Product Specifications do not belong in the Product Characteristic column but rather in the Product/Process Specification/Tolerance column. How are "Mixing of parts" and "Wrong label" being evaluated/measured? I realize you may have just thrown this together for sake of an example, but these ar a few of the things Control Plans get rejected by customers for (in my experience). In the Process Characteristic column you might have "Accept nonconforming product" or " Reject conforming product" as entries (there may be others). In the Product Characteristic column you would list the features on the product that you are monitoring (diameters, heights, widths, aesthetic properties, etc.). This doc is currently under review and if you have better format share it. Wanted to thank everyone for their posts, and information they have helped me out with my responsibilities of getting all of our documents and processes up to date. I wanted to know if any one has the PFMEA that goes along with the control plan that was posted by "pmwong". Our company has currently submitted PPAP for our customer and they have responded back that our Process Flow Diagram, PFMEA, and Control Plan need work. The posted control plan will help me out and just wanted to FMEA to go along with it. I also understand that they are linked together and you have to have one to finish the other. I just needed some examples to help me out to better understand them and to get my own in order. Also my company does plastic injection molding. I have found a couple examples already but they deal more with metal stamping.
