We've recently been supplied with some outscourced product from (sigh) China. QA, R&D and Procurement got around these parts and decided if they were OK, and what mods would be required before shipping to customers.
My manager asked if I would prepare a "product release form" to record such events - what decisions were made, and by whom etcetera.
Now I'm sure that this sort of thing has been done 1000 times, so I was hoping someone could direct me towards a document I could use to base my form around...

Mezz.It seems to me the format is not as important as whether you cover all the bases:

form
fit
function
delivery status (on time? packed right? count correct? paperwork correct?)
incoming inspection procedures (instrumentation? trained personnel? critical characteristics? sample count?)
schedule for periodic evaluation to determine everything is working or room for improvement
Good line of communication in case of glitch?
Control Plan for modifications before shipping to customers?
