Hi,
I'm a student of engineering and I must realize a fmea for a Hospital.
For you, Which scale is the best for S,O,D?
I've propose you my scale for S,O in attachement, but in italian.
Is there anyone help me? If there is, I can post the translation in English.

I send you a English version of my Severity and Occurence scale.
My work is very personalized because there isn't a lot of data, and occurrence scale is based on experience of doctors, nurses and technicians. This is the first attempt in this hospital.
I reada lot of post in fmea forum and I understand that fmea is flexible on organization in object.
I hope that It is a good rate.

Mattia, sorry for this delayed response. I was hoping a couple of our Medical Industry folks might chime in as I have not worked in that field.

The tables appear to me to be sufficient. The most important aspect of the FMEA methodology is that you have a ranking system which will focus you're activities on what is truly important and critical. If this "index rating table" makes sense to the organization and can be justified, by all means use it.
