I’ve run into a situation where I feel it’s a great idea to show my employees and MR’s what a completed NCR looks like. Can anyone share?

We’ve run into some issues where our registrar helped us write our QM, and now the auditors are asking to elaborate on a lot of these, when the answers are simple and won’t have system failure ramifications.

Welcome to the Cove! :bigwave:

Have you searched in the Post Attachments List? There is a very wide variety of forms, documents, templates, etc. posted there. It is the thin, green bar in the upper left hand corner of this page, beside the lighthouse called "Solutions".

OK...I'm searching through all 6,000+ of these, and I guess I'm the 1,000,000 person to ask, but there isn't a keyword search on the Post Attachment List???

What do you mean your registrar helped you writing your Quality Manual?

Maybe "write" was the wrong adjective. They advised us.

Except for all the admin stuff like date, location and that junk here is a very simple example....

The targeted objective of <8% scrap based on Production Sales has not been achieved for 6 out of the last 9 months of XYZ FY2006. There is additionally no evidence provided of any plans or courses of action to achieve the objective at the time of this assessment.

Here is one with a bit more detail....

The CA/PA process is not being performed in accordance with procedure XYZ 2.8.5001, Corrective & Preventive Action.

Though there is evidence of EMS CA’s/PA’s being addressed (ie EMS057) EMS CA’s/PA’s are not being entered into the CA/PA log Form 4.8.5001, responsibilities are not being assigned, response dates are not being assigned and root cause is not being identified.

Corrective action is also not being verified as identified through review of the CA/PA temporary forms and through review of identified CA for EMS056 closed on 2/13/2006 (Percentage indicated as a goal on the Internal rejects spreadsheet had not been changed to 8% from 6.3% as indicated in the CA).

Thanks for the guidance so far. I'm really impressed with how many people lend a hand to those of us ISO "newbie's." Without getting into a full blown gripe session about ISO, I now have a bit of reading to do.

The part that stinks is I'm not a newbie...this stuff can be a bit hard to "absorb."

My recipe for a good NC report is as follows:

Reference - usually a clause from the standard and/or the para of the procedure

Statement of fact - state clearly what the problem is. Phrases such as 'evidence of ....' or 'no evidence of .....' are usually good to use. Make sure to stick to the facts and don't make (or imply) generalisations unless you have evidence that what you have seen really applies to everything.

Objective evidence - list a couple of examples of the evidence you witnessed.

Scale of the problem - not essential but can add a degree of clarification e.g. 10 checked, 7 without references stated.

But what I was looking for was help on filling in the Correction, Root Cause, Corrective Action side. Reading through, are there usually redundant statements, or frequently used ones, etc.?
