"Hello,

We kindly request your help to collect data for our study about the use and spread of FMEA - Failure Mode and Effects Analysis as a major quality tool.

Could you therefore please:

1. Complete the survey on *** Dead Link removed *** (10 short questions), it takes less than 2 minutes to complete the questionnaire). In return for your help we will make the results of this study available to all (some results of a previous survey are already available).

2. Forward this mail to 5 (or more) people in other organisations. We hope to get 1.000 or more completed surveys by end of January 2000, this is of course only possible with your help. We will send a bottle of champagne to number 1000 ;-)

I don't remember hearing anything about the outcome of the survey. I came across this as I was going through old threads continuing to weed out dead links.
