Who knows how to maintain a simple electronic FMEA database to reflect a constant flow of "continuous improvement" changes?

Given the needs for "document control" and "history", simple editing is not acceptable.

Actually Lotus Notes (no personal comments please), although I guess the principals should remain the same.

You might look into ISOxPERT ($$$) from Management Software International. It is an add-on to Lotus Notes specifically for document history and control.
