Does anyone have or know about a database (possibly in access) which includes comprehensive ‎Information of suppliers for example information about : Presentation ,Cooperation history ‎‎,evaluation history and etc? ‎

I don't have a template, but our purchasing coordinator is currently developing one for our suppliers. It will be an Excel spreadsheet with columns for supplier's name & address, product/service they supply, ISO registration status, evaluation/audit history, FDA registration status, activity status, etc. This will help us track which suppliers are coming up for evaluation via survey or audit, current status (active/inactive), etc. as opposed to our current convoluted system.

Hope this helps. Maybe it will give you a place to start, at least. :)

I've always used Excel for similar databases and its work much better than Access. With Excel it is a lot easier to modify if you need to change your format (or pretty much anything) later on.
