I have been at it again! :D Apologies to all the Covers from outside the UK but some of the terminology is national. The HMRC is our equivalent of the IRS (plus some) - they managed to lose a couple of data discs with the bank records of 25 million UK subjects. I was asked to write an article for CQI's Quality World magazine about the problem and investigation.
