Can anyone help please? I am familiar with 8D reporting but does anyone out there have any examples of a 14D report?

what is the context? did a specific customer request this? did you see it referecned in an article?

I with everyone else here. I have never heard of a 14D...5D and 8D yes.

It exists alright. After much searching I found it amongst the Ford Motor Company APQP documents.
If I knew how to post an attachment i would have posted it.

I like to see a copy.

For a document, just use the paper clip icon, located next to smily. The rest is easy.

Well I hope that this works. I have uploaded it. It is a simple xl doc.

basically it supplements an 8D and asks for details of what went wrong, when and how.

Ford requires this when "you are in big trouble", like major spills, field actions, stop shipments, yard holds, losing Q1. (I have been through it and proud of it. lol).

It is usually presented to higher level management at Ford (upto VPs, depending on impact), prepared by the supplier, but often presented by the STA to his management. Also there would be a FASS review included.

I have more information, but I can get not get to it from where I am it today.

I have a 2004 copy of an FASS (Field Action, Stop Ship report) in my files and dig it up and share on Wednesday.

Here is the FASS report (from about 3 years ago). I deleted names and some other information. My recollection is that the STA took this to Ford management.

Thank you very much Kales for the information and example.

Fortunately we have not got to complete a FASS or a 14D as the concern is now downgraded to an 8D. We do not produce parts per million and have a Q1 waiver anyway.
Our supplier had sent us a faulty batch of parts and there was simply a lot of knee jerking going on initially.
However, it is interesting to know how the system works beyond the norm.
