How have companies registered to ISO 9001:2000 documented on-the-job-training as a fulfillment of training requirements and evidence of competency?

We came up with a one page form, detailing the training requirements, blocks to list the jobs they were trained under, and signoff for the operator and lead person.

We do similar to Carol; have a form used as Employee Skills Training matrix which identifies needs, that needs were satisfied or in progress, sign off by trainer. have one for each employee. A living document when new need comes along ESTR updated.
