View Full Version : Template for reporting Workplace Accident and Dangerous Occurrence. nidhir 1st April 2008, 06:10 AM Dear all, I urgently need a templete for reporting workplace aciident and dangerous occurrence. Thanks Nidhir Colpart 1st April 2008, 06:51 AM I am not sure where you are in the world but this may be of interest. It is from the Health & Safety Executive (HSE) website in the UK. It is from the Health & Safety Executive (HSE) website in the UK. Click here (https://www.hse.gov.uk/forms/incident/f2508.pdf) Colpart, Can you provide us with some more details about this form? Is it required to complete this? Employer, employee responsibilities? Very interesting. Stijloor. Colpart 1st April 2008, 08:07 AM Stijloor, I am a bit out of my depth with OHS but I think the employer is required to complete the form following an accident falling under the Reporting of Injuries, Diseases and Dangerous Occurrence Regulations (RIDDOR) 1995 see the HSE website for further (proper:confused:) guidance on the matter. SteelMaiden 1st April 2008, 08:39 AM Don't know if this is helpful, but here goes. Initially, a database entry is made with: Date and time occurred: Incident number assigned to this case: Supervisor: Specific location: Detailed Description Preliminary Cause Type of Incident: Immediate Corrective Action (Completed: No) Then, the incident is reviewed by our safety committee and cause determined (or verified) and long term actions are determined (counseling, something needs to be fixed, procedures need to be put into place) Also, at the time the incident is reported there is a drug test performed. Failure to report an incident is cause for dismissal. Randy 1st April 2008, 10:03 AM Steel has it...............All we're talking about is a basic accident report form Who What Where When How Why Can't get any simpler. nidhir 2nd April 2008, 12:44 AM Dear members, You have all been very helpful and I will use the uk hse reporting form as a guide and your comments to develop something specific to our needs. Thank you very much Cheers Nidhi:) |
