Thanks to all that are helping. I did the search and search this site but still not able to locate a simple form (like excel) to create a CAR for our company. We are a small company and I need to keep the form VERY simple to navigate and facilitate.

I tried to do a simple excel search and download from online, no result. Tried ISO, still nothing. I am not stupid but I have NO patience so I am most likely missing something obvious!
