Would someone please explain the differences between:

Changes that could affect the QMS
Recommendations for Improvement
Improvement of the effectiveness of the QMS and its processes
Resource needs

I do realize some are inputs and some are outputs. Can something be more than one thing? Can it be an input and an output in the same meeting?
For example, if it is decided in the MR to set a new objective or revise a current objective which element does it fall under.

Any help relieving my confusion would be appreciated.

Changes that could affect the QMS - anything that changes a process or practice that either revises or adds to your qms. ex.:automating a process. input
Recommendations for Improvement - anything you do currently that needs to be improved upon (or could be improved upon) ex.: using a database instead of a handwritten form, once the improvement is made, it will probably become a "change that could affect the qms" input
Improvement of the effectiveness of the QMS and its processes - the actual improvements made to the effectiveness brought about by activities performed due to the review of inputs. output
Resource needs - Management's resource procurement brought about from the review of the inputs. output

Could a new employee be both an Improvement of the QMS and a resource need? Then later a change that could affect our QMS.

I really think I am making this harder than it is supposed to be. The last 3 audits that I have had, my auditor keeps telling me it isn't being done correctly. I asked him what he suggested and he just quotes the standard, not much help at all.:(

you might be making it harder....

having the resource could definitely improve the effectiveness, I don't usually look at a person as an improvement, but more what they bring to the table. If it isn't being "done correctly" your auditor should do more that say it isn't being done correctly. He should offer you objective evidence that proves you are not meeting the requirements. :confused:

My review consists of a checklist for all the inputs to show that we discussed, along with notes about what we are going to do (if we are going to do something) outputs. Check the Post attachments link at the top of the page (green box by the site logo) for examples of what people are doing. The one I use came from perusal of same. Cannot remember which poster's was my inspiration, but gosh, I am thankful they took the time to share!

Why separate them into 3 categories. Instead, I would try to see them as supporting each other to demonstrate that the QMS conforms to requirements including continual improvement.

For example...

Change that could affect the QMS - ISO 9001 standard being revised to include new requirement under document control.

Improvement - Enhance current document control process to address all requirements, including new one.

Resource needs - Software to control software, temporary personnel to transfer documentation from company directory to software, personnel to provide training.
