Hi

I just wanted to know that is it mandatory to put/use reference no. in the check lists of internal audits?

They help, but not required.

It makes it easier to write any NC's found by referring to the paragraph of the standard.

I reference the internal procedures in internal audits. My NCRs are written to the internal procedure number. We have a reference document that cross-references the internal procedures to the standard (also to ensure that we have met all of the requirements), but the internal audit is to address compliance to the internal procedures. People should have a better understanding of the internal documents, and therefore will better understand the purpose/issuance of an NCR.

So, I agree a reference to something helps clarify things, but I don't use the standard's numbers.

Dude, its not even mandatory to have a checklist, you just have to have evidence that an audit actually occured...The record generated is the audit report with a specific retention period...after an audit I keep a bunch of notes in a folder with the date and name of the process, external registrar is fine with it.

Its not mandatory but it helps you a lot for easy identification where does a form comes from. On our end, the numbering is placed on the lower left portion of the form or checklist. Each department is responsible in assigning number for their respective forms. All forms shall have a revision number.
Example:
ABC-111 F-01 R-00
Where ABC-111 - is the document number
F-01 is the form number
R-00 - is the revision number.
