View Full Version : Non-conformance Root Cause Analysis Finding: Stupidity?
Aaron Lupo 2nd October 2001, 04:12 PM Has anyone here ever wanted fill out stupidity as the Root Cause for a Non-conformance. When the same error occurs time and time again what else can you say. I know that is not a valid root cause, but the situation will never go away unless the person leaves by their own choice. What are some of the other things others out there would like to be able to put down as the root cause. I could use a good laugh before our Recert. Audit in 15 days. :ko:

More accurate would be lack of Mgmt Support/Enforcement for a 'lasting' CA solution.

In the document section, I think, there is a thread called "I wrote the procedure and I goofed". It's full of suggestions and discussion on the very same thing. I printed out one section for possible use in the future.

I think that at least 5 times per year, one should be allowed to use, "Stupid" and/or "Usage of Bad Crack" as an excuse. I totally hear you on this one!! It just seems like that same freakin' people continue to make dumb mistakes.
I take the NCR to them and ask them how this happened. They just sort of shrug their shoulders and tell me this things happen. I just want to smack them upside the head- HARD! If I had the power I'd can their sorry little asses. But I don't, so I'll just fantasize about it here.:biglaugh:

I have no problem with the excuse stuff happens cause it does we all have bad days etc... but when it happens over and over and worst of all this person is one of our Quality Managers ugh!! I sit back and laugh to myself and just thank my lucky stars that i am not that stupid. I look forward to our Recert Audit it is the Department managers that don't. It will be a wonderful three days. At least I get luch for 3 days out of the deal.:D

Operator taken out back door and beaten soundly about the head and neck with a rubber hose. Hands not touched because he will be transferred to Janitorial Department.

And a true story....
We had an inspector that refused to accept the new Quality Manager and his way of doing things. We had some non-conforming parts that this inspector approved to "use as is" (without the neccessary approvals and authiorization). Customer rejeted parts, issued a CAR....inspector was fired and that is how we answered the CAR. A few weeks later we got a call from the customer, thinking our CA was too drastic until we explained that the issue was personality conflicts.

We have a similar story... Non-conformance on an annual compliance audit in the Purchasing Department. Former Purchasing Manager just didn't want to get with the program. So... we gave him the big Hasta La Bye-Bye. The auditor didn't want to accept that as a corrective action. The Exec VP got kinda pissed off and asked him:
1) Did it correct the problem?
2) Was it long-term and lasting?
3) Could the auditor come up with a better corrective action?
Needless to say, the auditor signed off on the corrective action...
:p
