I work for a software firm.

We are looking at upgrading our Complaints module and want to introduce some additional fields. These are:

1. Severity scale of the Complaint
2. The cost of handling the complaint
3. Potential risk associated with this complaint
4. Potential cost of this risk

Also want to move Root cause out of the corrective actions area into its own field.

We are looking for some complaint form templates so that we can understand what are common layouts. Particular interests from the pharmaceutical and automotive fields.

Alternatively we would be interested in any comments about the new fields we are looking at adding.
