Please, tell me, which clauses should contain an application for ISO 17025 accreditation ? I know, that every accreditation has its own form of application for ISO 17025 accreditation, but please, share with your experience about form of application for ISO 17025 accreditation.

You'll need to contact an accrediting organization such as NVLAP or A2LA. Discuss with them, and they should be able to help you begin the process.
