Does anybody have a document including the common paragraphs (or requirements) of the ISO 9000 and ISO 14000 systems ?

Appendix A of ISO 9001:2000 has what you need for ISO 14001 relationships.

Appendix B shows the relationship between 9001:1994 and 9001:2000

There are several threads here in the forums where this is discussed as well as examples of 'integrated' Quality Manuals (attachments to posts in threads among other places).
