Can someone provide a template that I can use to create FMEA for implementing six sigma in an organization? Any help would be appreciated.

I'm not sure I understand. Are you looking for something like an automotive FMEA? Like a Design or Process FMEA?

Not sure what exactly you are looking for. I believe there's nothing wrong with the 'standard' FMEA format. May be you need some kind of a software (not just a spreadsheet) that provides some powerful analysis facilities?

I would suggest you do a web search for tools such as C&E Metrics or Ishikawa diagram. www.isixsigma.com is good site for all info related to Six Sigma.
