I'm looking for models/templates for IS manual, IS policies, IS procedures, BCPs, Disaster Recovery, Risk related documents, Incident management and similar required documentation for implementing and certify an ISMS.

Sorry for the late reply. That's a lot! Whew! Thought you're gonna ask for a single document thats missing from your compendium. Anyway, turn your browser to www.sans.org. They got a nifty collection of information security policies there that would be a good foundation for you.
