I have not seen a standard six sigma format for reporting. I would suggest you look at some of the books on six sigma. The Six Sigma Way Team Fieldbook by Pande, Neuman, and Cavanagh (ISBN 0-07-137314-4) published by McGraw-Hill has some good examples of worksheets and forms that may help. Also look at www.isixsigma.com. They have a lot of articles, etc. that may also help.
