View Full Version : Quality System Ineffective - Fishbone Diagram the QMS System
noboxwine 20th August 2003, 11:28 AM ;) Greetings Quality Geeks around the world ! A slow day has drifted me into looking at some old techniques and today’s flavor is Fishbone. After looking at some old and very useful diagrams, I began to think that none of them seem to address what in my opinion is a Global Problem Statement: Quality System Ineffectiveness. So, after some more thought, another coffee and drawing conclusions on lessons learned, I came up with some Bones that may stimulate some thoughts on where we need to go in order to improve declining systems. Because of wretched commitment and an inept registration system, :ko: unless something drastic happens, I don’t see myself in Quality much longer. But as long as I am, I will try and offer solutions, no matter how odd they may appear. And, if they stimulate one tiny improvement, it was well worth it. Have a day ! :bigwave:
Kevin Mader 20th August 2003, 01:58 PM NBW,
Sorry to hear about (read about?) your frustration. I suppose that we all have felt our share of the same, but I commend your attempt to put a positive spin on things. Being in Quality is not for the faint of heart.
I'd have to start with Leadership as I believe that the faiure of a QMS (or business system) is born out of and inability to lead (there are other factors of course). Being at the head of an organization doesn't automatically make one a leader. And, even if what you find at the top is a leader, there aren't any gaurantees that this leader stands for something positive. But in regards QMS ineffectiveness, I put the responsibility of failure (or success) with leadership and then management.
What is the QMS about? What does it stand for? What is the Vision? So, for a bone or two, I suggest:
-Lack of Vision
-Lack of Commitment to the Vision
-Failure to create Commitment (Compliance is easy if you use fear or incentives, at least initially, but commitment takes longterm understanding)
-Wrong Leadership Style (i.e. Destructor, Procrastinator, Caretaker as defined by Crosby)
Well, here is a start.
Regards,
Kevin
Randy 20th August 2003, 02:59 PM 1st of all...BOX!! Can I use this thing in some of my course presentations? I'll give you credit of course. (But no money)
2nd....Kevin, you're sounding like me here. You'd better watch out or your rating will slip into oblivian like mine. I can do better though!
As far as I'm concerned most of the 'leaders" we see aren't. These folks (boys and girls, but mainly the boys) are manipulators that have climbed the rungs of success on the backs of others. These "leaders' are micro-managing, power poisoned, Tunopia stricken people that 1st and foremost put themselves at the top of the priority list. Without wanting to offend anyone, most of this crowd's parking spots should be in the handicapped area because they apparently can't hear, see or even feel, evidence bears this out. Takers?
3rd......most system management is done as a catch-as-catch-can type of operation. These things weren't built with value in them, therefore no real value is expected from them. These things are overhead, an expense, something that gets in the way of our doing business. Am I off traget here?
4th.....Hey Box!!!!! have you ever heard of Windgate Winery's?

Please use it. I'll send ya the Publisher file if you wanna customize it.
Not heard of the winery but just looked it up on the net. Next time I am around Pittsburgh, I'll look it up. Thanks for the tip. :smokin:

Well said as always Kevin. Frustration will soon switch to anger and then insanity if I think Quality will ever again be held in the highest regard. It's almost turned into a burden. But, the occasional consulting gig will be fun, as those companies are humble and truly want to improve. Too bad they are the exception !! :(
M Greenaway 20th August 2003, 04:00 PM Funnily enough I saw a fishbone just the other day on why TQM initiatives fail, I will post it when I find it again.
Claes Gefvenberg 21st August 2003, 04:10 AM Good idea boxer,:agree:
So what are we waiting for? Let's start adding data to that fish. It looks like it could need it. I'll gove it a shot during lunch.

Claes Gefvenberg 21st August 2003, 10:02 AM ....or this... I just started it. Anyone care to come with suggestions? Tell me what you want me to add and i'll post updates as we go along.

Good Stuff, Claes. Quickly, here are a few additions. Add, delete, adjust and fit as needed (more to come after a hapless meeting at 10 am):
Procedures: Not trained, not audited.
Standards: Misinterpreted, not read, understood or practiced as written.
Registration: No RIO, "do it for ISO, QS", not the company. Costly !
Improvement: Goals not defined or tracked. Content with status quo. Anchored by "old schoolers". "weve always done it that way"
Communque': Fear of embarrasment (saying wrong thing), underutilized elecronic media. Long & not to the point.
People: Management: Negative energy, control freaks.
Problem solving (great start !): Ineffective data collection / analysis, or conclusions drawn with no / little data.
Kevin Mader 21st August 2003, 01:53 PM NBW,
I found the same release when I consulted. Sadly, these are a spattering of happier times.

Randy 22nd August 2003, 10:41 AM Box & Claes.....Your stuff is now part of the Randy Daily Traveling Dog & Pony Show ;)
Mike S. 22nd August 2003, 10:48 AM Nice job, guys! Is that a turtle diagram?
One thing -- maybe under people > leadership > management, add "top management dumber than a box of rocks". Sad but true sometimes. :( :biglaugh:
Randy Stewart 22nd August 2003, 12:39 PM Mike,
Why are you insulting rocks?
Claes Gefvenberg 22nd August 2003, 06:32 PM Box & Claes.....Your stuff is now part of the Randy Daily Traveling Dog & Pony Show ;)
Wowie, we always wanted to be on a show, right Boxer? :vfunny:
Seriously: I consider that an honour... :agree: I don't think we're done yet, however.
Mike: I think we have the box of rocks cornered under People -> Management -> Leadership? I just... errr... phrased it in another way, so to speak.

Any changes will be covered by C&P action and continual improvement :vfunny:
Raffy 24th August 2003, 09:36 PM Hi Claes,
Thanks for the attachment.
Btw, are you using the mindmapper software on the diagram that you've attached?
Best regards,
Raffy
Claes Gefvenberg 25th August 2003, 04:14 AM Hi Raffy,
The software is a pretty old (v3.0) version of Mindman. See the mindjet site at http://www.mindjet.com

Raffy 25th August 2003, 09:01 PM Hi Claes,
I already view the attached url path, and its quite impressive to have an application for mapping a diagram. (e.g. fishbone diagram). I thought it was the mindmapper application that I've seen last week when I attend a seminar on Business Excellence.
Thanks again for the information.
Best regards,
Raffy
