Do any of you know if there are nay example audit reports posted to the forums? I have searched long and hard and have not been able to find one.
Ours is a pain in the ***. and I would like to see what others are using.

Attached are a couple of examples of audit reports. They are probably at the extreme ends of the spectrum. One is a simple fill-in-the-blanks form, while the other is a (much) longer freeform type of report. Obviously, what you want is a format that suits your needs - maybe something in between. I hope this helps...

One is a summary that is sent to all department managers. Another is the verification that is sent directly to the Presiden once the audit and corrective actions are comple.

I guess you meant an 'audit report' or post audit report because there shouldn't be anything to report before the audit (pre-audit).
Samples are found in our post attachments list - thin green bar at the top left-hand side of every page. Just key in a 'key' word to search for files you want. Thanks! Since it appears you only report out against the standard clauses, how do you report to show auditing processes? What criteria do you use for grading majors and minors? It will help others who may use this form to complete it correctly....
Bpoole 27th October 2009, 10:01 AM The report is just the report. When auditing, we use a copy of the process, which is a flow chart and a Internal audit guideline. ( attached). All become part of the audit along with any other relevant procdures or work instructions.

It seems that the guideline is a fairly basic set of questions and there are many other (supporting) processes that can be audited at the same time as the main process which is being focused on during the audit. Are the auditors required to develop more things to add to their list - like controls for measurement equipment, non-conformance controls, document and record controls, awareness, authority, competency of the people, etc??

That looks as though it would work fine as a 'reminder' of the main questions to keep in mind, and of course assumes competency on the part of the auditor/s. Do you use it to keep reports reasonably consistent?
Oops - ignore the previous question. I just read further and discovered the report template, which I didn't care for so much. Why oh why would you report against the clauses of the Standard? instead of taking a process approach to auditing (which your guideline seemed to indicate).

:confused: I'm confused what your post refers to. I really don't know who you think has 'ridiculed' you. I haven't seen anything remotely resembling this at all.
Are you looking for help? If so, have you tried searching first? Then, if you don't find what you want, perhaps start your own thread with the question you seek help for. You will always find that the more information you give and the more clear you are about what information you are looking for, then the more useful and the better the responses will be.
Are you unhappy about something that someone said?
