We have been using a lessons learned database for a while, but have a real issue with the quality of the data being entered. After reviewing some online examples (NASA and DOE) and borrowing some good ideas we've introduced the "Lesson Learned Statement" with the intention that this statement will be enough for someone to use in a checklist during product, process, system or design reviews. For example "Throttle bodies are supported by at least three mounting points based on weight." The situation that lead to this lesson as well as the specific solution is all additional information.

The hardest part is to get the lesson statement from the situation. Does anyone has any exercises, excellent examples or perhaps a set of questions that will help with this?

Would it help to have the Lesson statement more in a question format??? For example: "Are throttle bodies supported by at least three mounting points?"
