Drop Down ("Standard") Categories for Corrective Actions & Root Cause

I want to make a series of drop down lists for some home grown software for corrective action. I wanted to populate the root cause and corrective action with some "canned" ideas such as management system: training or education lacking or machine/equipment: inadequate design.

Does anyone have a comprehensive list? It may lead them down the wrong path for right now but they could use some categories. I know that they shoudl find the true root cause and not settle for the easiest option but without some hand holding the system will be stuck.

Categorizing main contributors to problems in drop-down menus may be helpful, as long as there will be determined and focused effort to identify the real root causes and not symptoms. There are instances where the root cause is very obvious, but in most cases, they are not.

I am still concerned that your user community will click the easy way out.

You might benefit from the AS9131 document, available for purchase from SAE (Rev. C of the document is forthcoming!!!). Attached is one page of the document for an idea of the different NC codes listed.

This will do nicely, I have some other lists but wanted to make sure it was pretty complete. I will use it for training and software build for the drop down lists. Just starting to get the organization to think about CA's. There is no requirement for it and most of the work is theoretical and research base. I anticipate pushback.

We have a list of H&S causes, but it doesn't contain all operative causes.
