Corrective Action vs. Failure Analysis - Corrective Action for Every Defect Found?

Ok, I am coming from a company that only dealt with Corrective Actions. (none mfg) Now, I am working for a mfg company that states they want a Corrective Action for every defect found. I am trying to teach them to start using the wording of "Failure Analysis" for the defects found and use Corrective Actions for other issues. Am I correct in doing this or is a Corrective Action the right way to do it? And now we are starting FRACAS (Failure Reporting, Analysis and Corrective Action System).... Can we have an 8D CAPA system and a FRACAS? In what ISO standard would the FRACAS fall?:confused:

Failure analysis is part of a corrective action process, and not necessarily a separate issue. The results of failure analysis should help you to determine whether CA is possible or feasible. How you structure the system of investigating nonconforming conditions and what you call the process is up to you. It doesn't help to have conflicting or redundant systems. ISO 9001 and its several offshoots (TS 16949, e.g.) all call for CA/PA processes.

:topic:Unless your organization is involved with extremely critical hardware, such as nuclear, space or Area 51 craft and unless your budget approaches the Pentagon military spending, I guess that strategy will not succeed.

Hello Shawn,

It may be helpful to establish a set of CA/PA criteria that help you to determine what course of action to take. It is not necessary to spend a lot of time and effort on a $10 problem. That does not make good business sense. When actual and potential nonconformities are discovered, they need to be reviewed, using the pre-established criteria, and then take the appropriate action. The problem is that the CA/PA process is often driven by emotion, resulting in overreaction, and possibly messing up a good process.

See if you can come up with some criteria. Search the Forums, and you'll find a ton of good stuff. If not, let us know, we'll help you.

Did you ask your company what they meant by "corrective action for every defect"? Did you ask what they wanted to accomplish by having a "corrective action for every defect"?

Your company's idea of a corrective action may be totally different that what we perceive. Once you have answered the question of what they want to accomplish, your questions may be answered.

There appears to be some confusion on your mind,when you say ....Failure analysis for the defects found and use corrective action for other issues.This is not an either/or situation.

For failures,you conduct 'Failure Anaysis'/'Root Cause Analysis' (RCA)/RCFA or whatever the name prevalent in your area;to be followed by corrective action(s) and preventive action(s).These are links of a single chain.There are many tools to conduct FA/RCA/RCFA and you may navigate through "Post Attachment List" button,provided at the top of this page.

I am attaching a few for 8D and Honda's 5P system,just to put you into the picture.You are welcome to return with more querries.
