Please Help! NCR (Nonconformance Report) - Basic questions - When should an NCR be filled out?

Hello

I do work for medium-sized manufacturing company. We're going thru ISO paperwork to get cert next Year.

I do have some questions about NCR system.

1. When NCR should be filled out?

If everytime:
2. Should I contact customer every single time NCR is filled out?
If yes - that would be a nightmare. If not - what is criteria that we should use to make decision to do that? Like functionality of product, safety etc.? And what if customer will see NCR and ask who gave me a permission to decide to let it go? For example I do have a hole which is 0.001" deeper than on dwg and I know how part is working and this does not meaning anything to part functionality, strength safety etc. Should I fill out NCR? Should I contact customer?

If not everytime:
3. What is the criteria that we should use to release NCR or not?
4. Who should decide about that?

I do have my opinion about that but I need confirmation
