Please Help! Audit Scenarios - your ideas and comments

Hello all,
I have 3 scenarios from audit perspective and I would like to get some ideas about them
A) In a manufacturing company, you find some of the final productions are damaged or not passing the quality checks. When you ask one of the labours he says that there is a machine which he believes that it’s not functioning properly and that’s why some of the products are rejected during the quality checks.
Now as an auditor do you raise an NC or you will need to look at more evidences? If it is an NC what is the relevant ISO 9001 clauses?

B) You as an auditor visiting a warehouse of a manufacturing company and you notice that there are some packages that are there for long time and not delivered to customer. When you investigate, they tell you that the customer refused the final product as he believes that it is not what he was requesting for. But the sales team are negotiating with the customer to solve the problem.
Now as an auditor do you raise an NC or you will need to look at more evidences? If it is an NC what is the relevant ISO 9001 clauses?

C) You are visiting an HR department of a company and you notice that recently they sent some employees for training. When you ask the HR manager “was the training effective?”, he tells you that he asked the trainees about the training and they said its very good and efficient.
Now as an auditor do you raise an NC or you will need to look at more evidences? If it is an NC what is the relevant ISO 9001 clauses?

Why don't you give us your suggestions as to the answers and we can agree or disagree with you as we interpret the scenarios?

I agree the above reply but still the comments as the below

---- an auditor should make further investigation, when the machine malfunction? whether equipment dept was informed for repairing? How the equpment was maintained ///// during waiting repairing time, whether the process specification need be changed? who approve this change? whether inspection was strengthened?

---- auditor should further investigate it, whether it is true the company did not meet customer requirement? whether they are rejected from customer's site, then check customer's requirements identification & review process, design/development process, production control process, inspeciton process,also customer complaint process

auditor make confirmation whether the HR dept only eveluate the training effectiveness by the verbal investigatio, if so then raise nc to 6.2,
the point is ensure person is competent.

Hi guys,

Thank you for your reply.
well a friend of mine had this LA exam and gave me those scenarios.
I am practising and learning this standard and cant answer those questions.
I have to learn the standard as fast as I can to work with my company for some external projects.
Cove is always an awesome place for my problems.
