SFI x PA (Suggestion for Improvement x Preventive Action) How to register them ?

Dear All,

Please comment if the concept is correct or not:

SFI = As there is no problem, there is no cause analyses
Ex.: An indicator used for scrap which goal is 0,5% and for 6 months the real value is 0,3. The goal could be reviewed to reflect real situation. That is a SFI.

PA = there is a potencial failure, so a potencial cause analyses must be performed.
Ex.: There is a tendency in the indicator of scrap to fall out of control (it´s on the upper limit, but not outside yet). A cause analyses must be done to define a preventive action.

My question is: do you use different forms for SFI and PA (due to the difference in cause analyses)? How do you register SFI?
