I am trying to check the effectiveness of the processes and raise NCR accordingly, this is because I am done raising NCR’s which are related to compliance issues because divisions/departments are finally almost 95% implementing procedures and policies and no issues regarding compliance.
That’s why I am asking guidance how to check effectiveness of process and examples of NCR’s effectiveness issue.

You could assess the effectiveness of the QMS processes looking at associated metrics and KPI's.
Each QMS process should have KPI's for effectiveness monitoring ( as well as in efficiency in ISO TS). Look at trends, check if the set KPI's have been met, on the contrary , assure that there are corrective action in place for associated shortfall.
