FPA (Failure Prevention Analysis) and FMA (Failure Mode Avoidance) info wanted

Could anyone give me an outline of a Failure Prevention Analysis and/or Failure Mode Avoidance process? Are they more or less the same thing? How do they tie in with DFMEA and 8D processes?

In what particular industry/business are these terms used?

My impression was that these terms came from automotive. The Ford FMEA Handbook mentions something called Failure Mode Analysis; from the brief description it sounds like it might be another name for Failure Prevention Analysis or Failure Mode Avoidance (if they are the same thing). But the Ford Handbook doesn't say much about what is actually involved in doing it or what the output is.

By the way, thank you moderator for clarifying the title of the thread.

I don't seem to be getting much response on this post, perhaps I posted in the wrong forum? I know that these are design tools and they work with the DFMEA and 8D; is there another forum that would be more appropriate?

However, I imagine they are much the same as FMEA and FMECA studies. Have you conducted a broad internet search already?

I wonder how the ‘scoring’ may differ from the conventional FMEA study, if at all??

Yes, I always conduct a broad internet search before I post a question here. There were a couple of items on consultants' web pages and a post from Miner two years ago.
