Safety Hazard Analysis Report - Need format or an example

Has anyone ever done one of these for a product? If possible I need the format or an example of what one is. We have to write one for our OEM and I'm not sure what goes into one.

Thanks but I'm looking for one you would do for a product you're developing this looks more like a workplace safety assessment.

You can use the same process (I have) my modifying the parameters.

Essentially identify what could go wrong (hazard), the liklihood (possibility) and consequences (worst case) of that hazaqrd bearing fruit so to speak. From that you can make an assessment.
