AS9100 Management Review NCR help - Output of the MR is not Adequate

I need help. We got a NCR of Management Review from AS9100 audit. NCR description is "Output of the management review is not adequate." Objective Evidence is Reviewed summary of 2012 management review, no records shown QMS process improvement decision was made.
Now I am not sure if the contaiment actions need or not. and how to do the containment actions about this NCR? Please give me advise. Thanks a lot.

I'm not really sure I understand your question, but let me try to give you some help. In our early days of ISO9001 we also were notified that our MRMs were not adequate. We changed each topic discussed to an ITEM. Each item was numbered and we had to have an Action for each item. Sometimes the action was "None" and sometimes it was quite involved and would take several months. Any time that there was an action required it was assigned to someone. That person was called on at the next MRM and asked for an update. Until the action was completed, it was reviewed at each MRM. Of course, all input and output was in accordance with the requirements of 5.2 of the standard.

How about the corrective action request(s) status?
- was it included in the MR?
-was there any CAR that went through a root cause analysis and that the improvement decision was made?

Containment action would be to conduct another management review and decide on one or more process improvement strategies.

Make sure that you understand what you got written up for.

5.6.3 requires certain outputs from management review. Another way of thinking about it would be that you are expected to generate action items from management review. The first topic, 5.6.3a is action items that improve your quality management system.

My reading of this is that should you have any, you need to record them. I also read it that you might not have any and that would still be OK.

"The inputs from management review shall include ANY decisions and actions related to a) improvement of the effectiveness of the quality management system and its processes . . . "

If you auditor was trying to tell you that you had to have some, he is mistaken.

However, it is my belief that if you had a thorough management review it is highly likely that you would have found some. Holding another management review to determine if there are some improvements that can be made in your system is not a bad answer.

What I find interesting is if the auditor actually reviewed the following:

• how output from top management review are flown-down into organization at operational level
• output from the management review: decisions to modify procedures, methods, training of personnel, hiring additional staff, additional process controls/inspections

I would request from the auditor or registrar to clarify this NCR (Which should have been done by the time the audit was done). You probably have the objective evidence if the auditor didn't write anything else in management review and you got nervous - it happens.

You could do 2 things - have another Management review and provide objective evidence or present to the registrar your documentation and have them evaluated for guidance.
