Problem Solving and Root Cause Analysis Internal Training - Examples to Share?

I am looking for some help!
I am currently gathering some information to enable a meaningful training course to be set up in my plant.
I am specifically looking for examples of good and bad 8D's, also examples of completed ishikawa diagrams, and 5 why analysis sheets and is/is not analysis sheets to supplement the material we already have internally.
I would be grateful to anybody who could let me have some examples out of their own experience

Hi Celtic,

The Cove lots and lots of threads on the related topic. Do a SEARCH and you will get valuable info.

One of my favourite threads for RCA is G8D File - A file for the search of Nonconformance Root Cause - G8D vs. 5 Whys

For the examples of good and bad, it would be more effective if you pick up examples from your in house documentation. I usually write a "desirable good" example to drive the point.
