Seeking: Nonconformance report form

I am looking for a good multi purpose Nonconformance report form. We do not manufacture at our US site but are covering our support processes for our OEM product line manufactured at Mexico site so they can get TS certified, they are currently 9001. I will not be using this form for Nonconforming product but more for nonconforming process output.
