Refer below definitions

Risk

Combination of the probability of occurrence of harm and the severity of that harm

[ISO/IEC Guide 51:1999, definition 3.2]

Hazard

Potential source of harm

[ISO/IEC Guide 51:1999, definition 3.5]

Three types of Hazards:

A biological, chemical, or physical agent that is reasonably likely to cause illness or injury in the absence of its control.

Biological Hazard

Microorganisms are the primary cause of biological hazards in foods eg, clostridium botulinum, is a strict anaerobe that produces the toxin that causes botulism, Listeria monocytogenes, E.coli o157.

Chemical Hazard

Food may be contaminated in the manufacturing plant with industrial chemicals such as cleaning agents, Sanitizers, pesticides and lubricants.

Physical Hazards

Physical hazards can enter in food products during any stage of the production or distribution process. Eg.. Stones, insects, wood, glass, plastic, metal fragments and etc..

In additional to what bio_subbu has suggested, you may also take reference from OHSAS:

3.6 hazard
source, situation, or act with a potential for harm in terms of human injury or ill health (3.8), or a combination of these.

3.21 risk
combination of the likelihood of an occurrence of a hazardous event or exposure(s) and the severity of injury or ill health (3.8) that can be caused by the event or exposure(s)

Examples:

Bacteria & viruses can infect the food resulting in infection/ food poisoning. Hence bacteria & viruses are hazards whereas 'food poisoning' is the resultant risk.

An unprotected live electrical cable is a hazard while 'electric shock' is the risk that may result from exposure to such a cable. The same cable may also be considered as a 'tripping' hazard in which case the risk would be injury/ fracture etc.

Similarly, you may consider 'manual handling' as a hazard which can result in severe backache/ muscle pull (risk).

In short, there's always a 'cause & effect' relationship between Hazard & Risk. Hazard is the cause while Risk is the effect.
