Medical device manufacturers often electrically isolate applied parts from ground and do not provide additional insulation. This was possible per 60601-1 2nd Ed. clause 17, in which minimum creepage and clearance distances were not required if the short-circuited device met maximum leakage requirements. It is not clear what happened to this rational in 60601-1 3rd edition. Review of the standard seems to deem the above devices as non-conforming.
