New Tooling Approval Process Acceptance Procedure - Injection Molding - Thermoforming

I am looking for a new tooling acceptance procedure. We make our own mold for injection molding and thermoforming operation. There are times we have to get parts approved by the customer for new design, but if we are making a mold for the previously approved part then it needs to be approved internally. Currently we have a very vague guide lines but not a standard procedure that everyone follows when new mold is made. I am looking to implement a process that takes place for each and every mold wether its a new design or an exixting one. This is from the quality management stand point, approval needs to come from engineeing, maufacturing and quality before tooling is handed over to production team. Anyone have any example or procedure that they can share.

Create a tool buy off form , that is applicable to new or repeat toolings.
