A procedure on how to fight loudly?

Just kidding - I don't know what the acronym is.

I think it is the end result after turning to run from an exploding grenade. But Randy would know better than me.

Failure Reporting and Corrective Action System (Think I got that right)

I am most familiar with it from Reliability & Maintainability and maintenance stuff. I went through Ford R&M - not sure if it is just a Ford term or not.

It is basically a corrective action for a equipment failure.

Failure Reporting and Corrective Action System? I vaguely remember hearing of this before as a CAR-type system for capturing and addressing field failures. Maybe with missile systems?
