Does any one know where I can get a good reference book on how to design a CAPA system?

I don't know of a book, but if you do a search here in the Cove on Corrective and Prevention Action you'll probably get some good links to some great information. Real world information...and that's better than a book!

I am charged with developing and implementing a new corrective action reporting system that utilizes root cause analysis. Where do I start?

It is entitled,"How to Design a CAPA system for FDA regulated industries"
It a little handbook with very good information it.It helped me a lot.
